A readable recap is not always an accountable action record. This prompt asks the model to keep decisions separate from discussion, attach a short source excerpt to each action, and show missing ownership or dates openly. If notes say “next call” and the supplied context gives that date, the prompt can record both; otherwise it keeps the original phrase rather than inventing a calendar date.
